Please use this form to apply for support for yourself or to refer someone else. The questions shown will adapt depending on the type of application you select.

We will ask for information about the person needing support, their circumstances, how personal safety technology could help and their financial situation. This information allows our trustees to consider whether the proposed support is suitable and whether funding may be available.

Please provide as much accurate information as possible. If you are making a referral, the person should normally know about and agree to the application before you continue.

Submitting an application does not guarantee that support will be awarded. Every application is considered individually and remains subject to our eligibility criteria and the funding available.
